Provides front line oversight to participants/members in the Diabetes Education Program assisting in the development of strategies to improve the health status of defined populations. Provides guidance and direction to program participants in achievement of specific chronic disease management objectives to improve health outcomes and status. Utilizes a number of population health management tools and analytics to provide education and resources so that program members are actively involved in the management of their own health and well-being.
The Licensed Physical Therapist Assistant provides physical therapy treatments to patients to facilitate increased independence and functioning under the supervision of a physical therapist licensed to practice in Mississippi. Performs all other duties as assigned.
Electronically places orders with vendors to maintain an adequate inventory of drugs and materials so that the Pharmacy can operate efficiently. Continuously evaluates and adjusts inventory quantities and par levels to provide optimum inventory and utilization. Maintains purchase records required by State/Federal law and hospital policies and procedures. Monitors purchase orders as they process through the system. Continuously updates drug cost information as required by the various hospital information systems. Verifies receipt and accuracy of all received orders. Provides reporting to monitor/evaluate all aspects of inventory management. Performs all other duties as assigned.
Functions under the direction of licensed nursing personnel to coordinate non-clinical tasks, including: transcription of doctor's orders, maintaining the patient chart and utilization of communication devices. Performs other duties as assigned.
