Overview and Responsibilities
Provides a wide array of social work services to patients and families, including performing psychosocial assessments, developing and coordinating discharge plans to insure continuity of care, and providing supportive counseling to enhance coping and functioning ability. May be asked to resolve issues occuring on all shifts, 7 days a week. Reports to Supervisor of Case Management/Director of Quality Case Management. Performs other duties as assigned.
Experience Description: None *** Experience Minimum Required: 1 year experience in a medical/clinical setting. *** Experience Preferred/Desired: 3 years of experience in clinical social work. *** Education Description: None *** Education Minimum Required: MS -- must be licensed -- LSW *** Education Desired/Preferred: MS -- prefer LMSW or LCSW *** Training Description: None *** Training Minimum Required: None *** Training Desired/Preferred: None *** Special Skills Description: None *** Special Skills Minimum Required: Advanced communication, conflict management, organizational and planning skills.
