Overview and Responsibilities
Improves patient's quality of life by providing comprehensive clinical services to patients in area of expertise to ensure optimal medication use and effective resource management, while considering pharmacoeconomic principles across the healthcare system. Works in collaboration with health care team members to accomplish goals. Performs other duties as assigned.
Experience Description: None *** Experience Minimum Required: Clinical knowledge and at least one year of direct hospital-based clinical pharmacy experience or the equivalent. *** Experience Preferred/Desired: Three years in active clinical practice in hospital pharmacy. *** Education Description: See Licensure *** Education Minimum Required: Bachelor of Science (B.S.) or Doctor of Pharmacy (Pharm.D.). *** Education Desired/Preferred: Post-graduate education in health-system pharmacy services. *** Training Description: None *** Training Minimum Required: PGY-1 Pharmacy Residency and/or equivalent clinical experience of three years. *** Training Desired/Preferred: PGY-2 Pharmacy Residency and/or Pharmacy Board Certification, BLS and ACLS certifications and/or equivalent experience of greater than three years. *** Special Skills Description: Computer literacy, interpersonal skills, general clinical awareness *** Special Skills Minimum Required: Advanced communication skills and computer skills, including Microsoft Office applications. High-level cognitive ability to interpret and assess written medication orders to ensure optimal medication use. Teaching skills and qualified to precept residents, students, interns and externs. *** Special Skills Desired/Preferred: Membership with state and/or national hospital pharmacy associations. *** Licensure Description: Current, active, unrestricted pharmacy licensure for appropriate state or eligibility for licensure. Must acquire licensure within 90 days of hire date. *** Licensure Minimum: PHARMACIST-FOR APPROPRIATE STATE *** Licensure Preferred: None
