Overview and Responsibilities

Provides leadership for the Integrated Care Management Department, Patient Placement and the Transition Program including department and program planning, development, staffing, budget, analysis, tracking, and reporting of associated clinical and utilization management data.
 

Provides vision and strategic planning for the development and expansion of care management related programs to support clinical integration, value based purchasing, transition (discharge) planning, and other health care reform related changes and initiatives.
 

Maintains continued focus on regulatory compliance, revenue cycle and cost reduction, including LOS, unreimbursed care, and the implementation of guidelines and protocols that improve clinical quality and process efficiency.

 

Qualifications

  • Experience Minimum Required: 5 years clinical experience with demonstrated leadership skills.
  • Experience Preferred: Experience in post-acute settings.
  • Education Minimum Required: ADN or RN Diploma
  • Education Preferred: Master’s in Nursing (MSN)
  • Training Minimum Required: Knowledge of health care issues, trends, legal issues, compliance and managed care with emphasis on discharge planning and care coordination.
  • Training Preferred: Disease Management experience in program development and design.
  • Special Skills Minimum: Ability to communicate effectively both verbally and in writing.
  • Licensure Minimum Required: ACM, CCM, RN in state of practice.
