Overview and Responsibilities

Job Responsibilities








 Performs a comprehensive psychosocial assessment of the patient and families response to the cancer diagnosis.
Works to maximize collaboration with the patient and healthcare team.
Identifies patient/family social/psychosocial issues that need to be addressed and provides support and education.
Identifies opportunities for education and program development of staff.
Demonstrates initiative toward performance improvement.
Completes assigned goals.

 


Job Summary








 Functions as a member of the cancer care delivery team in order to contribute to the development and coordination of the overall treatment plan. The goal is to assist patients and families facing the impact of a cancer diagnosis by helping them to identify social, emotional, and environmental needs and to connect them with available resources and services. Plans and implements programs and activities to support the goals of the cancer center and assesses program delivery and effectiveness. Performs all other duties as assigned.

Qualifications

Education













 Description Minimum Required Preferred/Desired
 
     Masters degree in Social Work from an accredited graduate school of social work. Computer skills to include MS Word, Excel, and Power Point. Excellent organizational skills.

        Experience














         Description Minimum Required Preferred/Desired
         
           Minimum of five years experience in health/medical setting and/or two years of social work experience in oncology. Experience working with individuals, families, and groups.
             Minimum of ten years experience in health/medical setting and/or five years of social work experience in oncology.




              Licensure














               Description Minimum Required Preferred/Desired
               Possession of current licensure or certification to practice clinical social work.
