Works to maximize collaboration with the patient and healthcare team.
Identifies patient/family social/psychosocial issues that need to be addressed and provides support and education.
Identifies opportunities for education and program development of staff.
Demonstrates initiative toward performance improvement.
Completes assigned goals.
|Functions as a member of the cancer care delivery team in order to contribute to the development and coordination of the overall treatment plan. The goal is to assist patients and families facing the impact of a cancer diagnosis by helping them to identify social, emotional, and environmental needs and to connect them with available resources and services. Plans and implements programs and activities to support the goals of the cancer center and assesses program delivery and effectiveness. Performs all other duties as assigned.|
